Struggling with Workers' Compensation Process After Work Injury and Poor Employer Response

I injured myself at work on Sunday the 18th, and since then, I've been struggling to get any clear answers from my employer about the workers' compensation process. This was my first time dealing with such a situation, so I wasn't sure what to expect. I reported the injury, but the response from my employer was rude and unprofessional. I filled out the necessary paperwork to the best of my ability and submitted it, but three days later, I was informed it was still incomplete. Apparently, a supervisor attempted to submit it without proper completion. I only discovered this when I returned to give my supervisor my doctor's restrictions after speaking with the safety manager. HR offered me a choice: take 10 days of unpaid leave or return to work and risk further injury. I feel like I'm going crazy trying to navigate this. I've called HR, but they haven't returned my calls, and my emails requesting copies of my paperwork have gone unanswered. It's been a week with no communication from anyone in management or HR.
