Seeking Practical Advice to Break the Cycle of Unemployment and Career Confusion

I've been struggling with my career for a while now and I feel completely lost. I'm 35 years old and have been unemployed for over a year due to a health issue. The frustrating part is that I keep finding myself in this cycle where I get a job, work for a bit, and then leave, only to end up back on the job market again. I can't help but question myself—am I giving up too quickly? Am I not grasping things fast enough? Or perhaps I'm just not finding the right work environment for me? This cycle has really taken a toll on my confidence, and I'm scared that at my age, being out of work for so long will make it even harder to find a new job. I don’t want to hear stories of people who overcame similar situations or get generic motivational advice; I need practical, honest advice. If you were in my shoes, how would you go about breaking this cycle? How would you identify the right job for you and figure out why you keep leaving positions? I genuinely want to create a realistic plan to get out of this situation.
