I was recently let go from my job in Nebraska after an incident where I damaged a company vehicle. The situation arose because I wasn't properly trained on the vehicle's dimensions, specifically its height. I take responsibility for not asking for this information and for not insisting on more training, especially since I had already received a warning for a previous incident where I backed into something at the warehouse.
